This Privacy Notice explains how Nursing Home Reporter (“we,” “us,” or “our”) collects, uses, and shares information when you visit nursinghomereporter.com and related pages (the “Site”), including when you submit a case-review intake form. 1. Who we are / important disclaimer

We operate an informational CMS-data report-card site and optional lead intake. We are not a law firm and do not provide legal advice. Submitting information does not create an attorney-client relationship.

2. Information we collect

a. Information you provide

When you use the eligibility / case-review quiz or related forms, we may collect: name, phone number, email address, facility name or selection, injury or concern type, approximate incident timing, resident condition, narrative summary, consent preferences, and any other fields you choose to submit.

b. Automatic and technical data

We and our vendors may collect IP address, device and browser type, pages viewed, referring URLs, approximate location derived from IP, and timestamps.

c. Advertising attribution

If you arrive from ads, we may capture campaign parameters and click identifiers (for example UTM parameters, gclid, msclkid, fbclid) and similar identifiers used for conversion measurement.

d. Public facility data

Report-card content about nursing homes is drawn from public CMS Care Compare / Provider Data Catalog sources and is not personal information about you, except where you voluntarily associate a facility with your intake.

3. How we use information

  • Operate, secure, and improve the Site
  • Respond to your inquiry and route it for a free case-review evaluation
  • Contact you by phone, text (SMS), or email when you have consented
  • Measure advertising performance and prevent fraud or abuse
  • Comply with law and enforce our Terms

4. How we share information

We may share information with:

  • Intake and case partners — including Ad Optimizer landing capture, Formspark (backup form storage), and case-management or law-firm referral partners who may review your inquiry
  • Service providers — hosting (e.g. Cloudflare), databases (e.g. Supabase), analytics, and advertising platforms (e.g. Google, Microsoft, Meta) when pixels or conversion APIs are configured
  • Legal and safety — when required by law, to protect rights and safety, or in connection with a corporate transaction

We do not sell your personal information in the everyday sense of a data broker listing. Some advertising measurement may be considered “sharing” or “targeted advertising” under certain state privacy laws; see Section 8.

5. Consent to contact (TCPA / marketing)

If you check the consent box on an intake form, you agree that we and our partners may contact you by telephone call, text message, or email about your inquiry, including using an autodialer or prerecorded/artificial voice where permitted, even if your number is on a Do Not Call list. Message and data rates may apply. Consent is not a condition of browsing public report data. You may revoke consent by following opt-out instructions in messages or by contacting us.

6. Cookies and similar technologies

We and our partners may use cookies, pixels, local storage, and similar technologies for Site function, analytics, and advertising attribution. You can control cookies through your browser settings; blocking some cookies may affect Site features or measurement.

7. Retention and security

We retain intake and related records as needed for the purposes above, legal compliance, and dispute resolution, then delete or anonymize when no longer needed. We use reasonable administrative and technical safeguards, but no method of transmission or storage is completely secure.

8. State privacy rights

Depending on where you live (for example California, Colorado, Connecticut, Virginia, and other states with consumer privacy laws), you may have rights to request access, correction, deletion, or a copy of personal information, and to opt out of certain targeted advertising or “sale”/“sharing” as defined by those laws. To exercise rights, contact us using the channels in Section 10. We will not discriminate against you for exercising privacy rights. We do not knowingly use the Site to collect personal information from children under 16 for sale or targeted advertising.

9. Children’s privacy

The Site is not directed to children under 13 (or under 16 where a higher age applies). We do not knowingly collect personal information from children. If you believe a child provided information, contact us to request deletion.

10. Contact / requests

For privacy requests or questions about this Notice, contact the site operator using the email or phone channel provided on your intake confirmation or the advertising materials that brought you to this Site. Please include enough detail for us to locate your records (such as the phone or email used on a form).

11. Changes

We may update this Privacy Notice periodically. The “Last updated” date will reflect material changes. Continued use of the Site after an update means you acknowledge the revised Notice.
