How to report nursing home neglect
Where families typically file complaints—and how inspection records fit into that process.
Start with the resident’s safety
If someone is in immediate danger, call emergency services. Ask for a medical evaluation and keep copies of findings.
File a state complaint
Each state has a survey agency that investigates nursing home complaints. Provide dates, names, and what you observed. Ask for a complaint number.
Contact the long-term care ombudsman
Ombudsmen advocate for residents and can help navigate facility issues. They do not replace a lawsuit or medical care.
Review the CMS report card
Look up the facility on Nursing Home Reporter for abuse icons, special-focus status, deficiencies, fines, and staffing. Bring that context to any attorney conversation.
