About Nursing Home Reporter

Nursing Home Reporter is published by Top Tier Legal LLC. We help families find and understand public Medicare nursing home records, including ratings, inspection citations, staffing information, penalties, and ownership filings.

How the reports are produced

We import government datasets and organize them into facility reports and local directories. Our explanations, record searches, and editorial ranking score help readers explore the source records. They are separate from official CMS ratings and findings. Read our sources and methodology for the calculation, coverage, and date limitations.

The site is independent of CMS. Public records can be delayed or incomplete and do not establish what happened to an individual resident. Information on this site is not individualized medical or legal advice.

Editorial standards and review

Every page is built from dated public records, and each summary sentence on a report card traces back to a specific CMS data point — ratings, deficiency counts, penalty amounts, staffing hours, or inspection narratives. When the records hold nothing for a facility, we say so instead of filling the page with general text.

Guides and methodology pages are written and reviewed by the Nursing Home Reporter editorial team against the source datasets before publication, and re-checked when the underlying CMS extracts are refreshed. Pages carry an updated or reviewed stamp so you can see how current the explanation is.

We publish what the records support and label the limits: a citation is not proof of injury, a lawsuit is not a finding of negligence, and a missing record is not evidence that nothing happened. If a page overstates the records, tell us and we will correct it.

Contact and corrections

For website questions or a possible record error, email contact@toptier.legal. Include the page URL, CMS provider number if available, and a link to the public source showing the discrepancy. Please do not send private medical records in a corrections email.

A correction to our presentation is separate from a correction to a government record. When the original CMS data is disputed, contact the agency responsible for that record.

Optional inquiries

The report cards and directories are free to browse. If you choose to submit an inquiry, the consent beside the form and our Privacy Notice explain how your information is used and shared. Submitting a form does not create an attorney-client relationship or guarantee representation.
